Subjects: First and last names and email addresses of individuals, team, department, or organization to receive feedback.

Respondents: First and last names and email addresses of those who will provide feedback. You may provide this information in an Excel spreadsheet.

• A word or phrase that describes how each respondent is related to the Subject (e.g., Manager, Peer, Direct Report, etc.)
• An abbreviation for each relationship type (maximum of 6 characters
• There is no upper limit to the number of relationships you can use

• These are the broad categories and specific items to include in the survey.
• Choose from our extensive library of over 1,000 items in over 140 categories or
• Provide your own list of competencies.

• Used to measure the Subject's performance on a given item.
• Select from the standard set or we can create customized scales for you.

• Optional open-ended, comment-only items that appear at the end of the survey.
• You can include any number of summary questions.
• We can even set up a project for you that consists ONLY of summary questions.
• We'll need a list of the summary questions you wish to include and the sequence you'd like the questions to appear in.
